Breaking down travel costs for a transplant abroad

Flights — obvious, but prices swing wildly depending on dates. Mid-week vs weekend can be $200-300 difference. I'm looking at 7-10 day stays so I need flexible return or a changeable ticket. Some clinics include airport transfers, others don't — that's another $50-100 each way if you're arranging it yourself.
Accommodation — this is the big variable. Clinic-adjacent hotels run $80-150/night. Airbnbs further out can be half that but then you're adding daily transport. Recovery means you want somewhere quiet, ground floor or elevator, decent blackout curtains. I'm budgeting 8 nights minimum in case I need an extra day before flying.
Food and incidentals — first 3-4 days you're not cooking. Restaurant meals, delivery fees, water, snacks, pharmacy runs for saline spray or extra gauze. I'm putting $50-75/day as a rough floor.
Companion costs — if someone comes with you, double the flight and food. Their hotel might be the same room so that's a wash, but their time off work is a real cost too.
Time off work — 10-14 days seems standard for desk jobs if you want the swelling and scabbing mostly gone. Manual labor or customer-facing roles need longer. That's unpaid leave for a lot of people, or burning PTO.
Insurance and paperwork — travel insurance that covers elective surgery complications is niche and pricey. Some countries require a medical visa or letter from the clinic. Translation apps help but having key phrases printed out for pharmacy visits saves stress.
Post-op follow-up — virtual check-ins are standard now, but if something needs in-person attention you're looking at another trip. Worth asking the clinic upfront what their policy is for complications after you've left.
Currency and payments — some clinics want cash (USD or EUR) for the balance on surgery day. Others take wire transfers but your bank charges $25-50. Credit card surcharges abroad can hit 3%. I'm bringing a mix and notifying my bank.
What I'm still fuzzy on: realistic recovery timeline for flying home (cabin pressure on fresh grafts?), whether clinics actually help with pharmacy runs or you're on your own, and how people handle the mental load of navigating a foreign healthcare system solo.
